Japanese is traditionally written/read vertically, from top to bottom, and each column is ordered from right to left.
But today in horizontal writing, the line is read from left to right (of course the line is ordered from top to bottom), the same as in English.
For example, the lyric subtitles on this MV is written horizontally, so you can read left to right.
Chinese and Korean have the same vertical/horizontal writing systems.

There are some websites I’ve found that have translated lyrics for many of their songs, including this one. You were basically right about it being about Ryo (guitarman) railing against the fans for valuing his image over his health. The last bit of the song where he’s screaming into the mic is basically him telling them off for it. Pretty ballsy to chew out your own fanbase in a music video, but you’ve gotta respect em for being true to themselves.
Love the reactions, I recommend Golden Guy by MTH if you can manage to find a copy of it.

Have you listened to The Gazette? They’re another japanese band, whose in rhe Visual Kei Genre when they first started oit oh… 15 or more years ago? They have a very diverse sound. If not I recommend Filth In The Beauty, was the first song I listened to by them back in high school and fell in love. Inside Beast, Dogma, Ugly are some more recentish songs. Ugly would be the heaviest vocal/melody wise, Inside Beast has some techno influences, Dogma is more rooted in the Visual Kei sound and has a wonderful MV.

(Also I love how MTH loves to troll their fans)

when i see this video, i see the guitarist ryokun after losing weight going to the most fatty and unhealthy ramen in japan. but before eating his meal, in his brain there are at least two mindsets trying to convince him; 1. the scene besides the poppy chorus telling him not to be unhealthy, and 2. the poppy chorus telling him to be unhealthy. even after the disco chorus transitioning to the metal riff chorus, the lyrics changes to ryokun saying “i am sorry but i cant be fat…” and so on. at the end when he shouts in the megaphone, he explains the health situation and risks in japan, but goes back to reality shouting “does it look like i give a fuck” and eats.
